Kidney Transplant Patient Navigator Program A Project of Department of Medicine

The Nephrology Patient Navigator Fund provides support for expenses of the Patient Navigator Program, which serves kidney transplant patients in the Department of Medicine at the University of Maryland School of Medicine.

The Patient Navigator Program, part of the Division of Nephrology, assists patients with appointment scheduling, medication review, insurance access resolution, laboratory services guidance, transportation resources, access to nutritional counseling, setting up physical therapy services, and financial aid applications.

How can you help?

Your gift to the Nephrology Patient Navigator Program helps provide education, direction, and support for kidney transplant patients.

The Patient Navigator works closely with the social worker, inpatient and nurse coordinators to help high-risk patients, many of whom are elderly and/or are from underserved populations. Your support of this program helps to ensure better outcomes for these patients after kidney transplantation at the School of Medicine. Thank you for your philanthropy!
